There must be several things conflicting with Far Harbor then because I'm not running Scrap Everything and still have a few distorted faces and multiple settlers. I'm guessing it's not Better Settlers because I could update that and everyone in the Commonwealth looks fine. They are working on a fix, but for now you can open the "Scap Everything - Far Harbor.esp" file in FO4Edit and delete the entire "Non Player Character (actor)" section and the problem goes away. Scrap everything does start the bug, I have noted that on a clean play then installing scrap everything. But if you use the raider overhaul is causes the bug to affect not only settlers and trappers... but it then causes the raiders and some random encounter npcs to then become glitched. In my experience so far... after the raider overhaul making the glitch worse and removing it and scrap everything most (not all) settlers return to normal, all trappers that are then generated are normal. But raiders, although appearing normal, all have the same face. Also using the two mods does cause the bug to exist in both the commonwealth and far harbor. I also use Raider Overhaul, but I haven't noticed a change in Trapper or Raider faces (though, that may be because I'm always blowing their heads off from a safe distance, and some like wearing crab traps on their heads...).
