Having an issue with VATS combining body parts

So like the title says...sometimes when I use VATS on humans(It seems to only happen on humans/humanoids), their body parts are sometimes combined. So like when I try to target their "Torso", it highlights their torso, both left/right arms and legs. And both left/right arms/legs have 0% hit chance even if I'm really close and they're completely in the open. I imagine this is due to some incompatability with some mods or my messed up load order or something, but I'm not really sure where to start. Any help/input would be appreciated.
