Few questions

What does cleared mean when you mouse over mark locations? Some of thee places I killed everything in don't say cleared but others most do and I thought cleared would mean everything dead.

 

In the inventory screen for weapons what does the little up arrow next to some gear mean? 2 of the exact same weapon with no difference in damage or price or anything but one has the up arrow and the other doesn't so it can't mean upgrade but I can't work out the meaning.

 

Using favorites and hotkeys for melee dual wield is fine, as long as they are the same weapon the hotkey lets me use the keybind to equip them. Moment I upgrade the weapons via smithing any time I try to use keybinds to equip the upgraded weapons only my right hand weapon will be equipped with the hotkey and then for some odd reason my keybind and favorite of the item disappears, why? Can it be fixed?

Usually it means that there is some quest objective or word wall or loot item in that location that hasn't been done. As areas respawn, they will still be marked as "cleared" but will have enemies and potentially dragons.

Might mean something such as attack speed or just a lighter material, or could be something you have a perk for (mace/sword/axe perks).

Not that I know of, have the same issue with favoriting them. Have taken to either enchanting one, or just leaving them slightly different improvement grades unless I just mix between axe/sword/mace.

Some of the things are small cairns with just 2 or 3 enemys inside and after killing it doesn't say cleared.

 

Nope 2 of the exact same weapon as I said like 2 glass swords with no upgrades at all and one has up arrow and other doesn't.

 

Yeah except hotkeys like that can only equip stuff in the right hand so for a dual wielder you have to manually open the inventory or favorites and click to equip the left hand whereas before you could double hit the hotkey for the weapon and equip both of them.
