I've made several races with four extra arms (they're Arachnid-based), but they are just painted to the existing arm bones in a couple of different ways - they move a little but certainly can't manipulate objects. This was enough for me because they're supposed to resemble spider legs, which lack opposable thumbs anyway.

 

I added the arms to the tongue slot. This slot can be used for all kinds of physical accessories, since it supports rigging as well as .egm, but it tends to screw up the facegen of races with which it is used. It would certainly be possible to add more humanoid arms this way but they still couldn't manipulate objects - at best you might be able to have them wave around.
