My game is chrashing when i attack

So my game is crashing when i am attacking nps in certain locations.

It only affects mele weponds not bow and range weponds..

I have no clue what's wrong and what to do.

 

It only happends when I attack, my companions can attack.

I'm having the exact same problem. It's also completely random as well, sometimes it doesn't happen, other times it does. Here's my load order.
