Anyone ever run into this bug before?

Bizarre bug I've NEVER had happen to me before, and I'm pretty sure it doesn't have anything to do with mods because I haven't installed any new mods at least a year and last year was around the last time I played FNV

The bug occurs at an undetermined period of time where the audio in the game on some channels stops playing entirely and won't come back, has anyone ever run into this before?

Which version of Windows?

Over the course of a year you would have several Windows updates. Have you checked to see if there are any updates for the audio driver from the manufacturer?

Have you checked to see if the problem occurs with both headphones and speakers?

Have you checked the Windows "Event Viewer" under "System" and "Applications" filters?

windows 7, automatic updates have been shut off since the advent of windows 10, I have no speakers

I'm not sure what this event viewer is, I don't seem to have 'application' filters

The "Event Viewer" is an "Administrative Tool" (under the "Start" button "All Programs" entry) that shows you Windows error logs. See this article or search the internet on "win7 event viewer".

Most likely is that "something" is turning off your sound in the game. That might be a mod hitting a condition, or something outside of the game running in the background. That's why you need to check the logs in the "Event Viewer". You can also check the logs in the game root folder for clues.

Edit: Almost forgot. As you are only using headphones, it is also possible that you have a broken wire in the cord, which works fine until it flexes just right to short out. This is a common problem with headphones, especially if they have been dropped or jerked or otherwise had strain on the cord.

On a different note: You don't have to have "automatic" Windows updates. You can configure it to notify you when they have been downloaded, but wait for you to install them. Every update has a link to a web page describing what they are fixing. Skipping updates is a risky solution as they fix problems and improve security.

Alright I checked event viewer and using filters found nothing related to FalloutNV in the logs

My headphones are in no way damaged as sound works fine in everything elxse, and sound works fine in Fallout New Vegas only to mysteriously disappear after a period of time

I'm also highly dubious it has anything to do with windows updates, as I haven't had updates for a while and I've never once had any issues related from that, I may try to update anyways to see if it works, and check driver software but...

At this point I think it's time to look at my list of mods

I'm going to do some more testing ingame to see if I can get some more variables to determine cause and effect

Me :)

but maybe is not the same problem, because this happening to me in NV, Fallout 4, Mass Effect 2 and 3.

Never find a proper solution, reboot the computer is the only thing that works for me.

If your sound is cutting out across a range of programs (games), then it is unlikely to be a problem with the games themselves and more likely to be systemic: driver or hardware.

"Cutting out" suggests it is working initially and only occurs after you have been playing awhile. That suggests either an exhausted memory issue, or overheating hardware. There is also the possibility that the interrupt used for the audio is also being shared with some other device that is only intermittently active, or some background app is grabbing the audio device.

If sound is immediately restored simply by rebooting, it suggests the problem is not overheating (as that won't generally cool off enough during a reboot), which leaves exhausted memory or shared audio/interrupt.

Frankly I would suspect a background app of grabbing the audio. Look for some app that sounds an audible alert, like an AV program or alarm clock.
