cant figure out these damned mods

trouble is im still completely new to modding, ive only done a few easy mods for new vegas

there are instructions for the quiet one but im struggling to figure out how it works, and the skull face one doesnt have any instructions

 

if anyone could help out a real newb that'd be fantastic :3
