Performance problems

Its kinda heavily modded, but right when i start skyrim it runs incredibly smooth. Its just perfect. But then suddenly (after anything between 5-60 min playing) it starts laggin rly bad. Sometimes it goes away but mostly I just save, quit and restart skyrim and its good again. I dont understand this.. Any ideas?

 

I have most popular graphics mods on with NMM.

Everything is on max.

 

System is:

i5 2500k cpu

8gb @ 1600 mhz memory

gtx 560 ti gpu
