weird fallout 4 issue

ok im having this issue

once i start the game and click play the loading screen comes up but the mouse cursor stays in the blue loading circle and once you left click or wait until its done loading it minimizes tot he desktop. you click on the program to enter in to it again the screen isn't centered but its cocked off towards the upper left of the screen leaving about 2-3 inch gaps on the right and bottom of the screen as black space. this also effects the mouse clicking making the cursor about 1/2 in from the on screen mouse cursor.

i have tried

uninstalling mods
uninstalling the game
deleting the pref file
deleting the mod manifest data

but the issue keeps coming back


i have tried to look this up as well. but it seems to be either a weird issue or a small percentile issue. because i cant seem to find it.

any help would be great.
