issue with ctd and lag spikes

I got a fairly big load order (167 esp/esm's) for Requiem skyrim and been having problems with lag spikes. I turned on the fps counter and it's been staying above 55fps, so I don't know how it could be lagging. Then when running from Marcarth to Whiterun, I start CTD'n near the end of the river to the east of Marcarth. I tried going around this area through the river and up the hill and keep CTD'n. I am not sure what is going on, can anyone help.
