Okay, my money is on one (or more) of the PerkusMaximus plugins. Assuming you use NMM, go to your plug-ins tab and disable all your PerkusMaximus plugins. Obviously, do NOT un-install any of the mods, -or you will thoroughly bollix your game. Anyway, once you've disabled those plug-ins, load up a save where you knew you were about to level up. Play and see if the effect persists. If not, you've now narrowed it down to one (or more) of those plug-ins.

Oh man I completely forgot I posted this before I went away for the weekend.

​I believe I managed to find out what it was! IF I'm right, then you get 2 perks and 2 to stats when you level through your major skills in Character Creation Overhaul, 1 perk and 15 to stats with minor skills.
