How do you use modifiers with Enhanced Hotkeys?

I want to bind spells with the ctrl and shift keys, such as using ctrl+1 to select a spell, but I don't know how to with this mod. Is it possible?

 

If no one knows, does anyone at least know of a hotkey mod that lets you use modifiers and the 9,0,-,= keys? The Improved Hotkeys mod, for instance, only lets you bind spells from 1 to 8.
