Skyrim Formulas?

So I've always wondered...what are the formulas that calculate the damage you deal in Skyrim?(You know,that fancy [Whatever]+[Whatever]=[Damage] stuff)

Some extra questions about formulas

-If you got +attack perk (The ones that boost your attack when you use a certain type of weapon by 20%-100%) go like this?;

[Perk]+[base damage]x[sneak attack]

Or

[base damage]+[Perk]+[sneak attack]?

 

-Does skill points without perk even matter?

 

-Same question for Alchemy and Enchanting formulas

 

-One time,I upgraded a weapon and it got to superior,and the next time I got a fine level armor,does formulas have anything to do with this?
