Is there a mod that pauses the game when looting, like in Skyrim?

No. But there is a mod for Skyrim that un-pauses the game when looting. Exactly like in Fallout.

am I wrong in assuming that it's technically too challenging to make one, or else it would have been here by now?

I assume nobody's bothered since you can technically do it in-game. You may just have to train yourself to hit a different button when you see that item list pop up.
