There is actually a "Sexchange" console command, which seems like the only way of going about it, but I haven't actually heard of anyone using it successfully...

It can be used successfully, but there are some significant issues related to doing so that make it almost useless outside of just screwing around.

 

Essentially, you may have noticed that when using it on the player, it usually crashes the game, or screws up the player's head/face. This is because the command doesn't work well on anything that is currently loaded by the game. Since the player is always loaded by the game, there will always be complications, although saving the game, then using "showracemenu" then loading your last save (without closing console or accepting changes) can usually clear up most of them in relation to the player.

 

As far as NPCs go, the same sort of thing happens, except that instead of just the face being bugged, the game simply crashes once they get re-rendered before being reloaded. Meaning that if you do it to an NPC you're looking at, the game will crash almost instantly. If you have an NPC who is selected, but is now in another space (an interior cell, or a different, non-loaded exterior cell) the NPC will be changed, but the game will crash unless you save, and reload the game because the NPC can't properly be handled.

 

The solution, without having to save, reload every time is to have an NPC selected in console who is not in your currently loaded cell, execute the sexchange command, then use "resurrect" to force the game to reload their model and dialogues (disabling them doesn't do it).

 

Regardless, many dialogues will either lose their voice, or still use the voice of their previous gender, and their hair and face will look the same. As said, this is more for just screwing around, and can't really be applied to any aspect of modding.

 

As far as setting up a mod, even if you alter the gender of the NPC in the CS, you end up with wrong looking faces and coloring, in addition to the dialogue issues, and doing so only works if the NPC hasn't been loaded in your game. Which, while the faces can be fixed, and the load issue can be worked around, can make such a mod more trouble than it's worth, given how little gender plays a role in the game.
