Reinstalled Windows 10. Game broke. Any recent popular problems?

I see.

Look, I'm not here to curse the work of others or raise false accusations. But once I had a crash problem with NV and nobody, NOBODY on the internet knew how to solve. I've tried everything you can think of.
The problem solved when I switched the NMM to FOMM.
It may not seem to make sense, but hey, this is NV, any slight ripple in the water can cause a tidal wave.
IMHO, at least it's worth a try.

do you remember that mental glitch where post-mortem-dismembered limbs get cloned every time I return to the cell?

With a "clean install" vanilla game? Got to be something specific to your system configuration, such as the video card/driver combo; but more likely something else as those are the obvious suspects. There are unexpected interactions between the video, mouse, and sound drivers. If you are using any "advanced features" of your mouse driver, try with them disabled. Otherwise consider testing with older versions of each.

You know, I've heard good things about FOMM over NMM. Same with MO over both. But I'm already on my way redoing my load order, so maybe if I run into more problems, that's what I'll have to do. I have this feeling it was either my very outdated version of OneTweak (which I had forgotten was why my game was in full-screen borderless in the first place, until I reverted to vanilla and had no option for it), or maybe that combination of graphics mods just was too much for some driver configuration that I'll run into again once I get to those. I'm doing graphics last this time, and will try not to go higher than 1K (I was doing 2K before, never 4K). The only other thing I can think of is that it's something to do with my audio drivers, which I'm remembering now were installed by my sound card itself when I installed Windows 10 again, and are probably not up to date.

About that bug, Dubious, that bug was first encountered on a different motherboard with a different GPU, CPU, RAM, and on-board sound. And now it's a new OS install with a new registry. If it's my mouse or keyboard drivers, well, Sherlock Holmes has a maxim about this.

I'll have a look at what advanced features are even available to me on this mouse, but this mouse and keyboard are about as basic as you can get.

Also, though I don't really ever use a gamepad for this game, I was using a 360 controller for this PC back then, and now it's an XBox One controller, so even that is different.

I did keep my personal files and executables, but nearly all my software is still uninstalled, and with a vanilla New Vegas, I don't know what personal files could possibly have this effect.

I did find that if a weapon is set to use multiple projectiles per use, any dismembered limbs are liable to be multiplied by that number. I think that may require the pinned limbs option to be ticked but I forget now. Point is, I don't know, maybe something to go on? It's a good thing I don't tend to dismember dead bodies unless I'm testing for this bug, cos I can more or less ignore it if the rest of the game works fine. It's just very perplexing is all.

Update: All my game-altering mods are back in, besides a couple of big ones that are less stable, like Freeside Open and NV Interiors, including a different borderless window mod that seems to have a good reputation. NV Interiors is almost certainly fine, so I'll add that back soon. The game is already slowing down a bit, without graphics mods, but I can say for sure that it's at the very least functional. Such a shame that NVSR doesn't work anymore, or all that stuttering would be gone I think. Incidentally, even before I started installing everything anew (but after installing Windows 10 again), the game now quits without freezing or requiring the Task Manager, and it doesn't keep running in the background if I use Alt+F4. It does hang a bit longer when quitting now that I have these mods, but if that's the worst problem I have, I don't mind.
