Things to check:

1. Find plugins.txt (Should be in this location: C:\users\"Your user name here"\AppData\Local\Fallout4\ . Also it should be read only).

2. Open it. It should look a lot like what you see in the plugins tab of NMM. If your NMM does not handle esl files, there may be some in there, if so no problem, just make sure the order is right. There will be an * by all active mods, so make sure NMM and the file agree. If NMM says active and plugins doesn't, close plugins for a moment and try saving your mod profile, that may fix it. If that fails, go to the mod in the mods list and temporarily decativate and reactivate it. Save your mod profile after.

 

If you don't have a plugins file something is wrong, try running Fallout 4 via launcher, check the mods section, you should see some there, check Creation Club too and make sure all your DLC and the texture pack are in. If this goes well, run Fallout 4 through NMM, see if all is well from thete, too.
