Multiple Insane Bugs

I have received such bugs causing me to teleport frequently (normally in battles), sometimes i transform into other creature such as bears (no polyform mod added), get stuck in the one position lasting up to 5 minutes where i am unable to open menu, change 1st person or use weopons but can only look around. Any ideas?

I'm no modding expert, but try disabling each mod one at a time and seeing if it continues. If you disable a certain mod and it stops doing those (or some of those) then you can be certain that mod is causing it or is apart of what's causing it.
