CTD When Fast Travelling or Leaving Diamond City

So I did some messing around with my mods (I moved all of my mods from my Hard Drive to my SSD by uninstalling and reinstalling), and after getting everything reinstalled, I've come upon an error where my game loads totally fine into my Diamond City save. If I try to load a save where my character is out of Diamond City, the game CTDs. If I load into Diamond City and fast travel out, or leave via the exits, I CTD. I have no idea what's causing the issue and could use some help. Here's my load order:

Edit: I'm able to load into another save inside Vault 111, but leaving the Vault CTDs as well.

Edit 2: Starting a new game CTDs once the opening cutscene ends.
