I was wondering if it's possible to have a shrine grant a blessing and a custom spell.

Yes. There's a few ways in which you could make that happen; more details would help.

I wanted to make a mod which grants you a spell you can cast when you pray at a shrine, but you lose it when you pray at a different shrine.
