BSOD is driving me to the brink of insanity SOS

This is basically going to be copy/pasted from a post on a tech support subreddit, but I figured this might be a more prudent place to seek help. I have reset overclocks, updated all drivers, run memtest, etc. I also ran a fresh install without mods and it seemed to work. I'm out of my depth at this point -- and am contemplating the unthinkable... playing vanilla. Any insight is welcome and greatly appreciated.

 

I'm attaching a picture of my mod list. I also have the unification patch, the one that merges the official DLC into 1 (for mod limit), and have used Mod Merger, which I'll also attach.

 

I just started Witcher 3 and have consistently experienced BSOD after ten or fifteen minutes of play. This only happens with W3 and I'm unsure what could be causing it. The BSOD occurs with the following code:
