Constant CTDs

Hi everyone.
I'm having constant ctds.
Before it was after some hours of gameplays and now it's after I start the game and load the save.
I have various crash logs in The Witcher 3 folder all about this type of crash:

Build: Not Set
Crash reason: EXCEPTION_ACCESS_VIOLATION (3221225477)

I have these mods installed:

All it's merged with Script and Mod Merger with no problem.
I have followed various ways to solve these kind of ctds:
verify cache
uninstall, clean and reinstall
administrator mode
change settings (borderless windows, fullscreen, fps ecc)

Still having these ctds. Dunno what to do to stop having these.

P.S. Recently I'm having the most CTDs at the Nilfgaardian Garrison in White Orchard
