GrindedStone
Well eh, plug anything new into the computer recently? It would be the same effect as having programs running in the background. So start with background programs disabled an nothing else plugged in. You start messing with mouse acceleration an Vsync it opens a can of worms. It's more easy to adjust your mouse in windows, then adjust the acceleration in-game. You can't get it perfect but it's useable. Moving the camera in the game an moving the cursor in menu mode are different. It's tough to balance those with what you use outside the game. The acceleration is to help with that, but we just turn it off. Try it enabled but adjust the setting in-game down one click. Then go adjust it in windows to get it useable in all three areas. You shouldn't mess with Vsync on Direct 3D games it just breaks them.
netarchy
The lip sync issue also happens with vsync enabled when using a 120hz monitor (thus vsyncing to 120fps) ;D I believe it is intended that the game run at 60fps for the lip sync to work properly, anything higher and it goes nuts.I personally despise running at 60fps, so I just deal with it. ChaChaLoco
Had this issue in FO3. You can keep V-Sync off, but you must instead go into the stutter remover (which you should have) .ini file and change the max FPS from 0 to 60. Anything above that will result in lip-sync issues, but FPS capped at 60 or below solves the problem.
