MadGax
One moment navmesh is fine, make no changes, fast travel away or reload a save you just did and all the sudden the AI decides NOPE, not going there anymore.......Rinse and repeat and the stairs that the AI decided was not there before is now there, but now another path is now gone.WTF!!Does this happen in vanilla also? DaughertyUSMC
Yur question raises additional questions, more than it brings any solution to mind atm. I need more info - specifics. To begin with, let's have a peek at your load order list. If you're using NMM, just use the copy plug-ins thingy and post it, otherwise head over to PC and locate your plug-in list @ C:/Users/AppData/Local/Fallout 4/Plugins, and post that here. I have, and still do in fact, major issues involving NPC AI (incl., but not limited to, my followers) and their lack-wit disability to get from point A to B, all the while hearing s*** like "What? Am I supposed to wish my self there?" Truthfully, it's been so long since I played anything close to resembling a vanilla game, I cannot honestly remember if I experienced much of this back then, and that was using PS4. Long since, I have migrated to strictly PC for any moddable games due to Bethesda's and Sony's spurious antics regarding scripted mod use. So let's begin the process of ruling out various culprits we DO have control over - our mods i.e. our mods list - we may have mods installed in common that could exacerbate the Navmesh issues. I know the vanilla Navmesh vs AI is garbage to begin with, but I believe it mostly worked ok, other than the falling thru floors (which is my strongest issue right now in my game) and standing on roof tops. What you have described more or less sounds mod-related, which is why I asked to see your load order. Lets start with that, first, then move forward from there.
