You can access the birthsigns and classes under the Character tab. To create a new one, right-click on the list on the left of the window and select New. Creating new classes should be self-explanatory, since it's basically the same as creating one in-game. Just make sure Playable is checked and Guard is unchecked. Ignore the Auto-Calc stuff.

Making a new Birthsign is a bit more complicated. In the Object Window on the left of the CS, expand Magic and click on Spell. Right-click on the list and select New. The Type drop-down box determines what kind of spell it is. Abilities are directly applied to their "owner" instead of added to the spellbook, like a Breton's Magic Resistance. The other two you'd probably be interested in, Power and Lesser Power, are self-explanatory. Same with most of the checkboxes.

To give it an effect, right-click on the effects list and select New. Giving it effects is basically the same as using a spellmaking altar.

After you've made your spell, open up Character/Birthsigns and create a new one the same way as a class. Move the Birthsign Window out of the way so you can see your Object Window, and click and drag your spell from the Object Window to the Birthsign Window's box on the upper right.

Birthsigns have a list of spells in the upper right corner of their window. Find your spell on the Object Window, make sure you can see both the object window and the birthsign's spell list at the same time. Move the mouse over the spell you wish to add, and press the left mouse button (Don't release). Move your mouse to the birthsign's spell list and release the mouse. The Skills a birthsign gives are just Fortify Attribute/Skill Ability Spells.
