It is altering the game in memory, not on disk. The changes will not 'stick' between sessions until you save the game with those changes. You have to be running both the trainer and the game to make changes. From viewing the video it does not alter the base layout. Primarily it allows you to add/remove various soldier stats and items, as well as base resources such as money, scientists, and engineers.
