I fixed my problem and here is how, but I have no idea how I ended up with the problem in the first place:

 

Uninstall everything. Including the mods.

Install everything again as per instructions and make backups as you go along just to be safe.

 

Whatever I had done corrupted something, I don't know what it was but the only way to get rid of everything was to salt the earth and try again.
