Using Lydia as an example...I just select her in the "Talk to Lydia" option, then scroll down in the dialogue and select "I need to trade some things with you" and she says very unhappily "I am sworn to carry your burdens" that opens up both of your inventories...I find the item I want to give and press R to transfer it. You can also left click with your mouse in her inventory to take things like her old armor and weapons. Typically they will equip items that are better than what they already have...so if Lydia is wearing leather armor and I give her Daedric she'll just equip and wear it...same with weapons, she'll use weapons stronger than what she already has in her inventory.
