He mentioned a hunger and blacking out, sure it could be a mental illness they all suffer, or it could be a more plausible (yet still not very) way for Beth to fit more "Cool" stuff into Fallout 3. If I remember correctly they are actually Cannibals but it's Vance that teaches/convinces them to only drink the blood and not eat the flesh. Ever since the medieval ages there has been a disease linked to vampirism simply because of the effects of the disease. Porphyria is a disease that causes light sensitivity, multiple forms of psychosis, and etc. The psychosis can cause the vampiric behavior... Systemic lupus erythematosus in another disease, called the werewolf virus... simply because it caused a rash that resembled the wolf's markings, and caused immune deformitie and psychotic problems as well. I think whatever loose basis there may have been in these has been long gone since the middle ages - any idea of incredible strength can only result from raging psychosis in a few very short-lived individuals. And I've no idea where the "immortality" thing came from, since people with porphyria or lupus typically have shorter life expectancies than their contemporaries. They definitely do not explain "the hunger" that the boy talks about when you talk to him. Even if it's not supposed to be fantasy, there does seem a mystical or supernatural implication to the way cannibalism is talked about by the characters. He preaches, near-constantly, about "their nature." He knows it's bogus, but he convinces them otherwise as a way of keeping them in line.
