Anyone have luck with Terminals?

Right, so, I went through all the proper channels and such for making a terminal, have it all set up, have the note made, however I can't select my note from the list. This is due to the fact that my note (call it TNote) appears under the Items->Note category and NOT Items->Note->Clutter like it should. Anyone know how to remedy this?

This is what I did:

 

I typed TERMINAL in the filter, and I found a basic terminal and right clicked on it. I chose EDIT, and I changed the item ID to (for the purpose of helping you) terminal01. Then I clicked OK and chose YES when it asked me to create a new FORM. I then right clicked on one of the forms with the red icon next to it, these are notes. I changed the item ID to terminal01note, clicked OK, and chose YES once again to the creating a new FORM inquiry. I then right clicked on terminal01 and chose EDIT again, and then my note showed up in the menu.
