Accidents that could be in a mod

ALCHEMY------when making a potion have some chance for a lethal explosion when trying a new combination of ingredients. The chances decline with higher skill but never go away completely.

 

SPELL CASTING-------- chance of a spell fizzle.The chances decrease with higher skill.

 

ARCHERY-------- chance of an arrow being fumbled or flying erratically. Chance declines with increased archery skill.

 

BLACKSMITH-------chance of getting burned,hitting your hand(what ever is doable in a mod) .The instance of accidents decreases with skill but never goes away completely.

 

LEATHER CRAFTING------chance of cutting and ruining a hide you are crafting..Instance of cutting decreasing with increase in smithing skill.

 

MELE-------chance of sword breaking/being dropped/fumbled ...which decreases with experience but never goes away completely.

 

STEALTH------chance of tripping or kicking a rock etc that could break stealth.Decreases with skill level.
