Character lvl, equipped itens and map mark deleted

i had this problem a few times and i'm really mad that it happened again

when i close the game and open it again i:

* i lose all perks and level of my character

* all marks on map are deleted

* equipped itens are deleted

* the level of my character is reseted to 1

 

 

*at picture are the few mods that i use

 

 

 

HELP ME PLEASE IS THE 4TH TIME THAT IT HAPPEN ='(
