The "malfunction" is utterly inconsequential other then in cut scenes. Why even bother putting in game randomly? So many utterly stupid design choices in this game. You'll be gagging from the malfunction one second then be talking completely normally to an NPC the next. Sometimes less than a second. Try calling Panam before leaving Rogues booth in Afterlife.
