Are you playing on "Hard Mode"? I don't, but understand some conditions have to be treated by a "doctor" to fully recover, by design.

Too many mods I don't know to be specific, but first suspects (obviously) are any making changes to "chems" (i.e. "MoreChems"). But I do see some "load order" issues that may be at least contributing. And I conclude you have not read the Nexus FNV wiki articles related to setting up the game from some of those choices.

* Did you install Steam to it's default location? (GOG is not a problem as it places the game correctly.) If so, please see the wiki "Installing Games on Windows Vista+" article for why the original default Steam behavior of installing games to the "C:\Program Files" or "C:\Program Files(x86)" folder tree was bad (they learned better, and don't do that any more but didn't change it for older games); and why "disabling UAC and running as Administrator" is NOT sufficient, with instructions how to move it. Among other things, later versions of Windows (since the game was released) force files in "system protected" folder trees to be "read only" regardless of how you set them manually. It includes instructions on how to correctly move the Steam folders out of that folder path.

Don't move it to a folder under "User\Documents" either, as "executable" files are not permitted to run from there for similar "anti-malware" reasons.

This is the single most important thing you can do to fix and protect yourself against problems in the future. As much of a PITA as that is, it's never going to be any easier than now. System updates often cause issues with games installed to these folder trees. With the game moved out of the default location you will not need to be running it as an "Administrator Account", which is safer. Please see the 'Restoring to "Vanilla"' section of the wiki "FNV General Mod Use Advice" article as well.

* "Compatibility patches" to make one mod work with another need to be placed after (as in "physically lower, higher numbered") in the "load order" than ALL of the plugins they are designed to make compatible. (This also applies to "fix" files.) ESMs should always be placed at the top (lowest numbered) positions in the "load order", followed by the related ESPs though they can be separated from their ESMs by other plugins. LOOT does a good job of determining those basic orders, but it is a "crutch". (The "Tale of Two Wastelands" site has A Guide for NOT using LOOT for those interested in learning how to manually manage their "load order" for themselves.) Getting the "load order" incorrect will cause "Missing Masters" errors as well. You can make minor adjustments to the order and tell LOOT how to remember them. It's in the on-line documentation under "Metadata".

FYI: The "pre-order packs" (aka "POPs": Caravan, Classic, Mercenary, and Tribal; available now in the "Courier's Stash" addon), are not considered "true DLC expansions" by most mods and are treated as "compatibility patches" to the base game because they just add certain unique items. So their absence should not deter you from using "All DLC Merged" plugins. Where they are included, they should come after "GunrunnersArsenal.ESM" which is a true DLC expansion. When in doubt, follow the process in the wiki "Missing Masters" article to confirm they are not "master files" for that plugin.

Specific examples I spotted as "questionable":

63 3f CustomLighting II.esp

64 40 CustomLighting.esp
80 50 Unofficial Patch Plus - Companion Suite (No Carry Reduction).esp
81 51 Unofficial Patch Plus - Companion Suite (No Damage Nerf).esp
82 52 Unofficial Patch Plus - Companion Suite (Vanilla Effects).esp
83 53 Unofficial Patch Plus - Custom Companion Suite Template - Copy.esp

Others may apply.

* Do not use an "Auto-purge" mod for game play. (That was old advice from before we knew better.) They are designed for debugging by mod creator's only, according to the author of the JIP extensions to NVSE. See the 'Issue: CTD without warning, "Out of Memory error", or stops responding after the Main Menu', "Cause-2", "Solution-3" entry in the wiki "Fallout NV Mod Conflict Troubleshooting" guide for the game settings that do the same job.

* If you have more than one mod that makes any change to the same record or set of records in the vanilla game, then you really need to use a "Merge Patch File" with that collection of mods. Please see the 'Third Rule: The Rule of One' and 'Merge Patch File' sections of the wiki "FNV General Mod Use Advice" article. When in doubt, create a "Merge Patch" file for your overall "load order" anyway. If you add, remove, or change the sequence of plugins in your "load order", don't forget to rebuild the "merge patch" file again.

I recommend anyone read the entire "FNV General Mod Use Advice" article to understand the differences between this game and others you may have experience with; especially if this is your first attempt to play a modded FNV or it's been more than a year since you last set it up. It is designed for someone who has never played a modded PC game before, so it tries to avoid making any assumptions, is kept "up-to-date", and covers years of "lessons learned" by a retired Technical Support expert. It is NOT a list of various mods to install that happen to work on the author's machine; only "the essentials". It addresses fundamentals underlying how to get the basic game and mods to work together. At least 90% of everything I post in the "Technical Support" sub-forum is already in that article. Once you can play a test game to at least Primm, only then should you try adding other mods from some other "guide".

Then it seems likely that "something" from a mod is continually pumping rads into you. Like a "cursed item", though I don't know of any. Unless it only seems to happen in particular areas, in which case it may be contaminated ground. (But people get some strange ideas as to what makes an "overhaul" appealing.) Most probably it's a piece of armor, so try removing items one at a time and "wait" for a couple of days to check if your rad count has gone up. Once you have determined a particular piece is the source, you can use the procedure 'Issue: Find the source mod-index of an object in-game' entry of the wiki "Fallout NV Mod Conflict Troubleshooting" article to identify the mod.

Otherwise you might try the procedure laid out in 'Mod Conflict Isolation' section of the same article. It's the fastest way to narrow down an unknown suspect mod.

droped everything and did a purge healing station no dice, and nuked the whole thing and let the game rebuild ini files still no

When you say "nuked the whole thing", do you just mean the save game, or the entire game folder tree including the INI files in the "C:\Users" folders? (There is no way for a problem which is not inherent in the game itself to persist through a complete "wipe and re-install of the game" unless you try to use the save game files from the previous setup, which would then mean the "change" got "baked" into the save game files.)

Please see the 'Restoring to "Vanilla"' entries in the 'First Timer Advice' section of the wiki "FNV General Mod Use Advice" article.
