Getting a lot of crashes & very unstable generally

Hope somone can help me out here, my game CTD's very frequently when entering new areas (going through doors w/ loading screens etc) and waiting or sleeping. Also doesnt like being minimized (Alt-Tabbed). Generally very unstable cannot play for more than half an hour or so without it happening.

 

I have tried launching from NMM and also the 4GB launcher

Any help greatly appreciated, can provide any extra info if needed
