How do I get Appearance Menu Mod to work?

Hello, I'm having trouble getting the Appearance menu Mod to work.

After the mod is installed, how do I change my appearance?

 

I'm not seeing the mod in the console. :confused:

I'd ask on the mod page, but the mod author has locked his comment section.

 

Any help provided would be much appreciated.
