I think an item you get from pretty much finishing the vanilla game. an example for the requirements could be: having all skills at level 100, finish all the main story lines, get all the steam achievements on a single character, play x amount hours, collect all dragonpriest masks and complete x amount of misc quests. Whether this item is a piece of armour or whatever that is up to the modder.

Sounds like something for Achieve That! Might want to check and see if it is already there or if it might be something the author is willing to incorporate.
