Daledge is right, there is no back story for this. Story wise, Yes he is a vampire, however if they made him an actually vampire game-play wise, it would be obvious to the player and thus no plot twist. He is meant to be thought of as a full vampire however (You just assume that he feeds every night) and the game makes a reference to how you won't see him out in the sunlight. As far as I can tell bethesda did not make him an actual vampire on purpose to allow the plot twist. While it seems intentional it does seem a bit lazy though

Well according to ingame lore, when you're at 25% Vampirism, people can't tell you're a vampire. When you reach 50%-100%, that's when people see what you really are.
So maybe he's just always at 25% Vampirism? But that wouldn't explain why he still doesn't look like a vamp at the cave.
