This happened to me suddenly. When using pip-boy to activate a mod (setting, config, conjure...does not matter) the mod removes itself and nothing happens. Also, when using the controller the mod no longer works. I have created the items on the chemistry set and attempted to active the mod and then it just removes itself and no action. Anyone else ever run into this issue? In middle of game and cannot proceed. Would appreciate any suggestions.

This just started suddenly. Was playing Fallout 4 when I access my pip-boy and attempt to activate a mod item (settings, config or summon) the mod item remove themselves from the pip-boy. I try to create the item on the chemistry set and activate the item on my pip-boy, again, it removes itself and does nothing! Anyone else ever have this problem. I cannot proceed in game until this is fixed, it affects so much of my current game.
