they don't really appear during the actual game at all.... you can only see them when on the inventory menu, and even if you fully undress and exit the menu, you have an underwear on during gameplay, so yeah, it was only marketing, like "hey look at our game, you can even customize your genitals"

+1, im really tired of pulling weapons outa my ass it's really stupid that they left something like this out of the game... i mean, instead of taking examples of mods and improving the system, they just left it to the modders, it's just lazy

uh, but it's already possible to equip him with bandanas... even helmets and armors just put the item in his inventory and press T to equip it, or whatever button you assigned to it (i mean, just read the damn texts lol)
