Figured out the issue, in case anyone is wondering, under the quickbms.exe properties/General click the "Unblock" security option, and under Compatibility, select "Run as Administrator". Just installed about 10 mods, all work. When copying new mod files, don't overwrite the quickbms.exe.

I'm trying several different mods, some are installed in content0, others in patch0. I've tried Ultra Gore, and Over 9000, as well as several others, but I really want these 2. I copy the files, I run the batch installs, and everything comes back like it ran fine and reimported, but nothing changes in-game. I've even granted full permissions to the entire folder structure of the game, but that didn't do anything.
