I recently had this problem in my playthrough, and, I know this is three years late, but make sure your hunterborn is loaded BEFORE Realistic Needs, but your hunterborn patches loaded AFTER your RND patches. That's what worked for me, at least

When I was at full health, I didn't die.

I had this problem too, and I'm almost two years in the future. I have no idea what's causing it, but it might be an issue with moonlight tales.
