Follow the link in the OP for the appeal form. It will allow you to log in to file the appeal.

Log out of everything, forums and the sites. Close your browser,, relaunch and sign in again to everything. This will set a new session cookie that will allow you to access the premium servers. Buddah Closing the browser is essential.
