I know this is months later but I am having the exact same issue. I just randomly seem to have a heart attack and fall down dead, then the game reloads the last save. This is especially weird considering I use Death Alternative which is supposed to shove me into a bleedout state instead of death and save reload. I don't suppose anyone else has found a culprit or a fix for this?
