This would probably be quite a big mod but essentially if your intelligence is lower than 4 you actually get a downside of being mentally handicapped. Reason it would be a big mod is because you would have to redo a lot of ways people interact with you and probably add in custom voices especially for the main character. Things like some characters pitying you etc. Idk it would just be very interesting to see it be brought back.
