If you are using UFO, you may be able to recapture her by going fast traveling or going in and out a load door near where you last remember seeing her.3. Have you checked back at the Bannered Mare?4. Use the console command <set playerfollowercount to 0> This will remove all your followers, who *should* return to their original location. If it keeps happening, you may want to have a good look at your mods. But I think we have all lost a follower now and again. It was way worst in the early patches...
