Very Frequent CTDs, No Idea Why, Please Help!

As the title suggests, I can play for maybe only around 20 minutes or so before I get random CTDs, and it is quite annoying. I cannot enjoy the game with such frequent crashing. Here is my current load order in hopes of finding a corrupt mod or a possible answer. Please help, as I would love to be able to play without constant crashes and worrying where it'll occur next.

patch is the merged patch I created using FNVedit, and the new equipment, lever shotgun stuff, etc. are just some custom weapons by me that only have some edited stats and are standalone.
