We tried to build a new NPC, and for some reason, her face has "tear-lines" around the mouth.

 

These show in the CK and in-game. Also, her eyes also seem to have an extra white layer that looks like an inner membrane.

 

We never had this before. Where did we slip up?

This is not a texture issue. It can happen, if you use too extreme mouth settings during character creation. Try to adjust mouth/face slider settings and/or try a different mouth shape.

Not exactly sure, what it could be.

 

Option 1: Wrong or missing eye texture. Open your follower's head mesh in Nifskope. Find the eye mesh, click through to "BSShaderTextureSet". Have a look at the third slot from above. It should link to a subsurface texture (something like "eyebrown_sk.dds"). Add that texture, if it isn't already there. If you need such a texture, you can get it from the Bethesda Texture BSAs (you'll have to unpack them first) or from an eye mod.If it's not the "eyebrown_sk.dds", check your other eye textures.

 

Option 2: Eye glitches can occur, when you convert a head mesh from LE to SE. This is NOT a bug with programs like "Cathedral Assets Optimizer" or "SSE nif optimizer". There are some things that simply can't be automated.
