This will reset load order but an easy way to check if you have orphaned pak files that are causing the problem is to Purge mods in Vortex and then check the Mods folder (%APPDATA&\Local\Larian Studios\Baldur's Gate 3\Mods). This should be now empty. If it isn't, then there are PAK files there that Vortex hasn't installed and this is causing the bug. You can safely delete whatever is in this folder and then go back into Vortex and press Deploy. This will then copy all of Vortex's managed mods back to where they should be.

 

Sorry this is a bit annoying, the new update is close to being released and is sitll being tested. Feel free to try out the pre-release version and let us know how you get on. Quicker we get more eyes on this, the quicker we can roll it out to everyone.

I am having this same issue.  Mods were working fine in the morning (even a custom-edited one I installed via Vortex's Drop Files tool), downloaded a few mods to add to the pile, tried to re-install and bam, every single mod had this: "[mod.pak] couldn't be read correctly.  This mod be incorrectly locked/unlocked but will default to unlocked."  Even the old mods aren't active now; none of the mods are working in-game but the BG3 launcher still thinks the game is modded (it gives me the 'mods detected' popup).

Tried purging all my mods (verified app data mods folder is empty), verifying integrity of game cache, then re-installing all my mods and still no joy.  Tried un-installing the latest mods before it broke: nothing, still throws the error.

In between a non-modded game and getting the mods originally working, I had this strange bug where the game wouldn't launch via the launcher; I had to manually launch the exe but none of the mods were active.  The only way to get mods to work was to launch through the launcher app.  I fixed it by purging/redeploying mods, but now none of my mods work at all and can't figure out why.  Not sure if that means anything.  I'm running the Vulkan version if you're curious insomnious.
