I would first of all recommend slowing down the timescale in the game, as time will pass by too quickly, and a lot of the times (like in the case of the real hunger mod) you will get frustrated because you will have to end up eating quite often. Heres how to change the timescale(scroll down or search for timescale):

I guess realism can factor into roleplaying. For that I would definately recommend getting Cobl (common Oblivion) it's great for a lot of different little things. You can check it out here:

The choices and consequences and enhances quest roleplaying mod adds more choices for you to choose from when doing quests and such:

Persuasion overhaul is nice as it takes out the persuasion minigames and gives you dialogue choices instead (seems a bit more realistic): http://www.tesnexus.com/downloads/file.php?id=12367

You could also grab personality idles, but I found that a lot of the animations were kind of annoying, like when talking to certain npcs, they keep their heads down (in the beggar stance). But otherwise it's a nice mod.
